Motivated by a strong belief that facilitating partnerships is the best way to address community needs, CCHN staff actively participate in numerous state, local and regional coalitions. These groups meet regularly to assess need, implement programs and interventions, and facilitate community building around emerging health related issues. Some of CCHN’s partners are small grassroots organizations and others are large established nonprofit entities.

P2(PURSUING PERFECTION) COLLABORATIVE OF WESTERN NEW YORK, INC.
The P2 Collaborative is committed to the health of the Western New York community. This nonprofit organization is recognized as a change agent and works effectively with community groups and networks of groups like CCHN to identify and promote healthy lifestyles. Their current partnerships with CCHN focus on quality improvement, provider reimbursement, and chronic disease management.
HEALTH FOUNDATION FOR WESTERN AND CENTRAL NY
The Health Foundation for Western and Central New York is committed to improving the health and health care of the people and communities in their regions. The Foundation focuses primarily on frail elders and young children living in poverty. Further, their added focus on growing health capacity within these communities, and concentrating their resources on 16 specific counties, maximizes this impact. CCHN works with the Foundation as a community partner in an effort to strengthen the health care system, bolster community supports, promote education and advocacy, and encourage individual behavior change.
WESTERN NEW YORK RURAL AREA HEALTH EDUCATION CENTER
The Western New York Rural AHEC (R-AHEC) serves as a dynamic link between the resources of teaching schools and the most critical health and program needs of our rural communities. It has been specifically structured to insure maximum community based input and accountability. CCHN has worked closely with R-AHEC to increase middle and high school students’ awareness of health care career options.
THE CHAUTAUQUA CENTER (TCC)
Through the efforts of CCHN staff, representatives of the Community Health Center Association of NYS, the Community Health Foundation of Western and Central New York, Congressman Higgins’ staff and a multitude of other committed individuals and organizations, Chautauqua County is now home to a Federally Qualified Health Center. The Chautauqua Center provides comprehensive primary care services to low income, uninsured, underserved and otherwise vulnerable populations and facilitates access to a full range of needed medical behavioral health and dental services as well as case management services.

HEALTH CARE ACTION TEAM (HCAT)
Convened by the City of Jamestown’s Strategic Planning and Partnership Commission, HCAT is working on a common recruitment strategy for physicians and other health care personnel to Jamestown and the surrounding area. CCHN director Ann Morse Abdella is an active member of this important team.
