Jim Sherry, MD, PhD
Executive Director
Dr. Jim Sherry is the Executive Director of Chautauqua Health Network and is responsible for the oversight of the organization and as well as all associated programs and initiatives. Additionally, Dr. Sherry is a Clinical Professor of Health Policy and Management at the Graduate School of Public Health and Health Policy at the City University of New York. Dr. Sherry has extensive experience in policy, program and project design, implementation, and evaluation experience in low- and middle- income countries and communities.
Kenny Macdonald
Director of Operations and Communications
Kenny Macdonald is the Director of Operations and Communications, and his role includes providing office support for staff including oversight of budget, invoicing, resources management and payroll. Kenny is also responsible for developing strategic communication policies to ensure efficient internal collaboration and reliable information promulgation with partners and the public.
Jenny Rohlin, RN, BSN
Technical Lead for Community Health Development
Jenny Rohlin is the Technical Lead for Community Health Development at Chautauqua Health Network. At CHN, Jenny leads project development, management and program assessment initiatives to address social determinants of health, quality and performance monitoring, and population health initiatives. Jenny has been a registered nurse since 2012 with hospital, home care, and public health experience as both a staff nurse and department leader and is a certified lactation counselor.
Jason Carmen, PCMH CCE, CPH
Practice Development Manager
Jason Carmen is the Practice Development Manager for Chautauqua Health Network with six years of experience. A former Army Medic, he has won broad renown from area practices for his support and positive working relationships that he has spent a great deal of time to develop. Jason is a Certified Content Expert in the NCQA Patient-Centered Medical Home Delivery Model.
Kalan Brown
Data Analyst
Kalan Brown is the Data Analyst and has a significant history working in data development and reporting solutions, working in analytics with Chautauqua Health Network since January 2011. In his role, Kalan works directly with health care providers in the practice and hospital setting providing technical assistance, improving quality data and reporting, reduction in HCC coding gaps, and maximizing reimbursement rates. Kalan has experience with EHR adoption, meaningful use, disease registries, and workflow redesign.
Jabneel Dunn
Community Health Outreach Coordinator
Jabneel Dunn is the Community Health Outreach Coordinator and has experience working both in primary care and hospital settings and is currently pursuing a nursing degree at Jamestown Community College. Jabneel is responsible for assisting with the oversight of implementation, coordinating and facilitating network meetings, and utilizing a collective impact approach to conduct program evaluations.
