CCHN actively participates in state, regional and county health planning initiatives. The NYS Prevention Agenda serves as the blueprint for state and local community action to improve the health of New Yorkers and address health disparities. A goal of the Prevention Agenda is to prevent health problems before they occur, or before they worsen. The New York State Public Health and Health Planning Council’s Health Committee established an Ad Hoc Committee to lead the development of the 2013-2017 NYS Prevention Agenda.
Enormous effort went into the development of the plan from over 200 individuals representing local health planning agencies from all over the state as well as NYSDOH staff. The Committee assessed the progress of the Prevention Agenda 2008-12, examined the current health status of New York State’s population, defined overarching goals and new priorities for the 2013-2017 period, and obtained stakeholder feedback on the proposed priorities. Stakeholder committees were established to develop strategies to address the priority health issues.
The 2013-2017 NYS Prevention Agenda Priorities include:
- Prevent chronic diseases
- Promote healthy and safe environments
- Promote healthy women, infants and children
- Promote mental health and prevent substance abuse
- Prevent HIV, sexually transmitted diseases, vaccine-preventable diseases and healthcare-associated infections

Regional Planning Coalition
Funded by a grant from the New York State Department of Health, the Western New York Community Health Planning Institute (WNYCHPI) is dedicated to improving the health of (and health care in) our region by supporting population-based health planning to achieve appropriate alignment of health care resources.
WNYCHPI is the planning arm of the P² Collaborative of Western New York. They coordinate a collaborative, ongoing, sustainable, region-wide health planning effort within the eight counties of Western New York (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming Counties), as well as align and provide supportive tools to assist communities and their stakeholders with their heath-planning efforts.

CHAUTAUQUA COUNTY
The Chautauqua County Health Planning Team is composed of representatives of the area hospitals, Chautauqua County Health Department, CCHN, and The Resource Center. They identify health priorities from a number of perspectives as the hospitals develop their Community Service Plans and the Health Department develops the Community Health Assessment.
CHAUTAUQUA TAPESTRY
Chautauqua Tapestry brings together service providers, families and the community to share responsibilities and resources to enhance the delivery of services and improve the lives of youth and families. This organization provides an opportunity for Chautauqua County service and support providers to partner with youth who have emotional and behavioral challenges and with their families to create an accessible, responsive, appropriate and effective service delivery system.
